Abstract

Background
Atopy usually develops in time starting from skin (atopic dermatitis) and gastro-intestinal (food allergy) conditions and evolving to respiratory conditions (rhinitis and asthma).

Aim
To study the outcome of the atopic diseases (AD) in paediatric patients treated at the Homeopathic Clinic of Lucca (Italy), and the clinical evolution of children with atopic dermatitis, allergic rhinitis and asthma after a period of about 8 years, and compare with a group with conventional treatment.

Methods
An observational longitudinal study was done on 551 paediatric patients, (25.7%) of the 2141 patients consecutively visited at the Homeopathic Clinic of the Hospital of Lucca from September 1998 until December 2008. The Glasgow Homeopathic Hospital Outcome Score was used to assess the outcome distributed according to a Likert scale from -1 to +4, defining different degrees of improvement.

Results
The number of children with atopic diseaseswas 213 (38.6% of paediatric patients): 83 (15.0%) affected by asthma, 51 (9.2%) allergic rinocongiuntivitis, 76 (13.8%) atopic dermatitis and 3 (0.5%) food intolerance. Patients with follow up were 104 (48.8%), 65 of them (62.5%) reported a major improvement (score = 3) or resolution (score = 4): 22 (52.4%) patients with asthma [12 (48.0%) of which with allergic asthma], 17 (63%) patients with allergic rinocongiuntivitis, 25 (73.5%) with atopic dermatitis, including urticaria. Parents of AD children starting homeopathic treatment at age≤4 years were invited for a re-evaluation 8 years later and till now 28 children (mean age 11.6) have been examined. Our data were compared with a control group of 77 children with AD under conventional therapy from the age of 2 was invited to a follow up visit 9 years later at the Allergy and Clinical Immunology Unit, Meyer Paediatric Hospital, Florence (2001). Only 35.7% of the homeopathic group still had a mild form of AD vs 46.8% in the control group. Moreover the homeopathic group of children with AD at age≤4 developed asthma 8 years later only in 14.3% of cases, compared to 42.8% of children with conventional treatment (p = 0.003).

Conclusion
The results tend to identify a positive therapeutic effect of the homeopathic treatment in paediatric patients with atopic diseases, particularly in atopic dermatitis and allergic rinocongiuntivitis. Preliminary data showa tendency of children with AD treated with homeopathy to retain atopic dermatitis to adult age, and a minor tendency to develop asthma.
